Don’t forget to fill out the Illinois 5Essentials Survey.

Help Blythe Park reach our goal of 20% parent participation! *The survey takes less than 5 minutes to complete! Blythe Park needs at least 20% of parents to participate in the survey for results to be calculated. Currently there is 6% parent participation.

TO TAKE THE SURVEY, click on the following link and select the appropriate survey to begin: 5Essentials Survey

The Illinois 5Essentials Survey is designed to allow parents to share their thoughts on the effectiveness of Blythe Park School in the following areas:

  1. Effective Leaders
  2. Collaborative Teachers
  3. Involved Families
  4. Supportive Environments
  5. Ambitious Instruction

Your participation in the parent portion of the survey will help District 96 understand the conditions at Blythe Park and guide improvement. *Your identity and survey responses will be kept completely confidential and will never be connected to you or your child.
